Recent Advances in Proctology

HPV Infection & Anal Cancer Risk

In recent years, it has become clear that HPV (Human Papillomavirus) infection is a major risk factor for the development of anal cancer, particularly in:

  • HIV-positive individuals
  • Immunosuppressed patients
  • Men who have sex with men (MSM)

These groups are especially vulnerable to precancerous lesions, which can progress to cancer if not treated early.

The Role of High-Resolution Anoscopy (HRA)

High-Resolution Anoscopy (HRA) allows for early detection and treatment, significantly reducing the risk of cancer and improving outcomes.

Based on the ANCHOR study findings, early intervention with HRA:

  • Prevents lesion progression
  • Improves prognosis
  • Lowers morbidity in high-risk groups

New Techniques and Therapies

Modern proctology benefits from advanced, minimally invasive techniques that reduce surgical trauma and recovery time.

Hemorrhoid Treatments

  • Laser Therapy (LHP): Shrinks hemorrhoids without removing tissue. Quick recovery, minimal pain.
  • HAL-RAR: Ligation of hemorrhoidal arteries and lifting of the rectal canal. Long-term relief with low discomfort.

Fistula Treatments

  • FiLaC: Internal laser closure of fistula tract. Minimal tissue damage.
  • LIFT: Ligation at the fistula base. Preserves sphincter function and avoids incontinence.
  • Mucosal Advancement Flap: Covers fistula with healthy tissue. Effective in complex or high fistulas.

Pilonidal Cyst Treatments

  • VAAFT: Video-assisted endoscopic cleaning of pilonidal sinus. Less pain, faster healing.
  • EPSiT: Endoscopic cleaning and wall rejuvenation of the cyst to promote healing.

HPV and Anal Cancer Prevention – A Clinical Priority

HPV remains a critical factor in anal cancer development, especially in vulnerable populations. HRA allows:

  • Timely treatment of precancerous lesions
  • Reduced cancer risk
  • Improved long-term prognosis

HRA is a key tool in early detection and intervention, especially for high-risk individuals.
