There is no risk of abscess formation or generalized thrombosis following hemorrhoidal thrombosis.<\/strong><\/p>\n\n\n\n<p><strong>There is no surgical procedure that allows patients to remain on the toilet for more than five minutes.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Conservative Treatment of Hemorrhoids<\/h2>\n\n\n\n<p>When hemorrhoids are diagnosed, the first step a patient must take is to correct their bowel habits. It is inappropriate and ineffective to undergo surgery while still using a mobile phone on the toilet.<\/p>\n\n\n\n<p>Correcting defecation behavior is both essential and effective prior to any decision regarding surgery. If strict adherence to these habits fails to improve the condition, then a more interventional approach can be considered.<\/p>\n\n\n\n<p>Simple skin tags protruding from the anus do not require removal or treatment.<\/p>\n\n\n\n<p>Thrombosed hemorrhoids are very painful swellings caused by constipation, prolonged sitting on the toilet, intense physical activity, or exhaustion.<\/p>\n\n\n\n<p>There is no danger of thrombosis spreading to the heart or abscesses reaching the testicles due to hemorrhoidal thrombosis.<\/p>\n\n\n\n<p>What is truly needed is rest\u2014not surgery.<\/p>\n\n\n\n<p>The feeling of incomplete evacuation, more common in women, refers to the sensation that a bowel movement wasn\u2019t fully completed, affecting quality of life. It may be due to hemorrhoids or internal rectal prolapse and requires a different treatment approach.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Conservative Treatment Is Usually the First Line for First- and Second-Degree Hemorrhoids<\/h2>\n\n\n\n<p>In such cases, symptoms are usually mild and can be managed through lifestyle changes, diet modifications, and local care.<\/p>\n\n\n\n<h2 class=\"gb-headline gb-headline-57e28ba7 gb-headline-text\">Everyday Habits to Avoid Surgery<\/h2>\n\n\n\n<p>Before any surgical procedure, patients should commit to certain lifestyle changes that may reduce symptoms and prevent progression, thus eliminating the need for surgery. These changes include:<\/p>\n\n\n\n<p><strong>Diet and Daily Habits<\/strong><\/p>\n\n\n\n<p>Constipation is one of the primary aggravating factors for hemorrhoids. Therefore, <strong>dietary changes are essential<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>High fiber intake<\/strong>: Diet should be rich in fruits, vegetables, and whole grains to ensure soft stool and avoid constipation.<\/li>\n\n\n\n<li><strong>Adequate fluid intake<\/strong>: Drinking plenty of water daily helps prevent dehydration and constipation.<\/li>\n\n\n\n<li><strong>Avoid prolonged toilet sitting<\/strong>: Time on the toilet should not exceed <strong>5\u201310 minutes<\/strong>, as pressure worsens hemorrhoids.<\/li>\n<\/ul>\n\n\n\n<p><strong>Local Hygiene and Care<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Avoid soaps and standard toilet paper<\/strong>, as they can irritate the sensitive anal area. Excessive cleaning should be avoided.<\/li>\n\n\n\n<li><strong>Warm sitz baths<\/strong>: Daily warm water baths help relieve irritation and pain. Soaking for <strong>10\u201315 minutes<\/strong> can significantly reduce inflammation.<\/li>\n<\/ul>\n\n\n\n<p><strong>Medication<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Creams and ointments: Topical treatments containing corticosteroids or anesthetics can relieve pain and inflammation.<\/li>\n\n\n\n<li>Anti-inflammatory medications: In cases of severe pain, pain relievers and anti-inflammatories may be used.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"gb-headline gb-headline-092c4fb5 gb-headline-text\">Surgical Treatment<\/h2>\n\n\n\n<p>Hospital-based surgical procedures are only indicated for <strong>advanced cases<\/strong>. A high volume of hemorrhoid surgeries by certain surgeons may indicate an <strong>aggressive treatment approach<\/strong>.<\/p>\n\n\n\n<p>According to international data, hospital admissions for hemorrhoids in Greece should not exceed <strong>4,000 annually<\/strong>.<\/p>\n\n\n\n<p>The traditional hemorrhoidectomy (Milligan-Morgan) has largely been replaced by <strong>less painful and bloodless surgeries<\/strong>. Many effective surgical options now exist. Over the last 20 years, around <strong>20 different devices and techniques<\/strong> have emerged (Longo, HAL, THD, Rafaelo, Laser\u2013LHP, etc.).<\/p>\n\n\n\n<p>A specialized proctologist should be able to perform <strong>4\u20135 of these techniques<\/strong>, choosing the right one\u2014or a combination\u2014based on the patient\u2019s <strong>clinical condition and personality<\/strong>.<\/p>\n\n\n\n<p>Some surgeons promote <strong>trendy or exotic procedure names<\/strong> for marketing, which do not always reflect superior outcomes.<\/p>\n\n\n\n<p>The <strong>appropriate treatment<\/strong> is chosen <strong>after detailed proctoscopy<\/strong>, often involving patient effort to simulate defecation, revealing hemorrhoids that are not visible at rest. In rare cases, the patient may be asked to defecate to reveal full prolapse.<\/p>\n\n\n\n<p>Some hemorrhoids are treated with <strong>rubber band ligation<\/strong>, others with <strong>laser<\/strong>, and some require <strong>surgical excision<\/strong>. Each technique must be applied <strong>strictly based on its indications<\/strong>. Preoperative counseling for proper bowel habits is essential.<\/p>\n\n\n\n<p>Techniques like <strong>laser<\/strong> and <strong>ultrasound-based treatments<\/strong> (either alone or in combination) provide excellent results in selected cases.<\/p>\n\n\n\n<p>Laser is ideal for <strong>small external hemorrhoids with mucosal prolapse<\/strong>, while <strong>HAL-RAR<\/strong> using ultrasound is best for <strong>internal, bleeding hemorrhoids<\/strong>.<\/p>\n\n\n\n<p>Rubber band ligation is suitable for select <strong>second-degree hemorrhoids<\/strong> and yields excellent results. Pain after ligation typically results from <strong>poor technique<\/strong> or <strong>inappropriate candidate selection<\/strong>.<\/p>\n\n\n\n<h2 class=\"gb-headline gb-headline-dbfa0ac0 gb-headline-text\">When Is Surgery Necessary?<\/h2>\n\n\n\n<p>Hemorrhoids <strong>do not always require surgery<\/strong>, and it is best to treat them when they are <strong>not in an acute state<\/strong>. Surgery during inflammation can lead to <strong>more traumatic recovery<\/strong>.<\/p>\n\n\n\n<p>Surgery is <strong>only necessary<\/strong> when:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hemorrhoids bleed heavily, especially if anemia is present.<\/li>\n\n\n\n<li>Hemorrhoids prolapse either occasionally during defecation or permanently.<\/li>\n\n\n\n<li>Fluid discharge is present from the anus.<\/li>\n\n\n\n<li>Thrombosed hemorrhoids during the <strong>first 2\u20133 days<\/strong> cause unbearable pain.<\/li>\n\n\n\n<li>Skin tags are painful, cause hygiene issues, or are removed for cosmetic reasons.<\/li>\n<\/ul>\n\n\n\n<p><strong>Key Rule<\/strong>: Time on the toilet should not exceed <strong>5 minutes<\/strong>. Otherwise, hemorrhoids will recur after treatment.<\/p>\n\n\n\n<h2 class=\"gb-headline gb-headline-ae8425b3 gb-headline-text\">Treatment Options by Hemorrhoid Stage<\/h2>\n\n\n\n<p>Hemorrhoids are classified into stages and forms, but these <strong>don\u2019t always reflect severity<\/strong>.<\/p>\n\n\n\n<p>For example, <strong>some fourth-degree hemorrhoids require no surgery<\/strong>, while <strong>third-degree ones may demand urgent treatment<\/strong>.<\/p>\n\n\n\n<p>The patient\u2019s <strong>personality and pain sensitivity<\/strong> should be considered.<\/p>\n\n\n\n<p>Painful methods are <strong>not suitable<\/strong> for sensitive patients, while others may choose <strong>definitive treatment regardless of pain<\/strong>.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>First-degree hemorrhoids<\/strong>: Rarely need treatment.<\/li>\n\n\n\n<li><strong>Second-degree hemorrhoids<\/strong>: Treated with <strong>sclerosing injections<\/strong> or <strong>rubber band ligation<\/strong> if a stalk is present. If the dentate line is involved or there\u2019s no stalk, <strong>laser<\/strong> is preferred.<\/li>\n\n\n\n<li><strong>Third and fourth-degree hemorrhoids<\/strong>: In circumferential prolapse or large hemorrhoids, <strong>HAL\/THD ultrasound techniques<\/strong> or <strong>laser<\/strong> are preferred.
